• ID Pics 1x1 (6pcs.)
• Proof of Billing /Remittance
• Latest Two Months Payslip

• ORIGINAL NSO Birth/Marriage Certification 
• 24 Months Updated Pag-ibig Contribution
• Certificate of Employment/Job Contract(OFW)
• TIN/Passport(OFW)
• Cedula
• SPA (IF NEEDED, especially for OFW)
• MSVS (to follow upon seminar in PAG IBIG)
• Post Dated Check
